- Request access to records (PDF) Use this form if you would like to ask for records from a public body. The Office of the Information and Privacy Commissioner does not receive and process these requests. You can submit a request for records online here, Request access to records Online. - Request for Review (PDF) or Request for Review Online Form: Use this form if you would like the Information and Privacy Commissioner to review an issue in any of the following situations:

- You requested access to records, and you want the Commissioner to review a decision, act or failure to act, that relates to your request,
- You were notified by a public body that you are a third party, and the public body intends to disclose your information, or
- You believe that your information has been collected, used or disclosed in contravention of the law.
- Report a Privacy Breach under the Health Information Act (PDF) Use this form if you are a custodian, and discover a breach of privacy. The Health Information Act requires that custodians notify the affected individual(s) and the Information and Privacy Commissioner at the first reasonable opportunity, unless you believe that the breach will not have an adverse impact on health care, or on the affected individual.
- Report a Privacy Breach (PDF) Use this form if you are a public body, and discover a breach of privacy. This is a voluntary notice if you would like advice from the Information and Privacy Commissioner on managing the breach.
- Authorization to Represent (PDF) Use this form to delegate an agent to represent you in a request for review with the Information and Privacy Commissioner Office.

Alberta FOIP: Focus on Privacy - This interactive online course takes one to two hours to complete (at your leisure) and will give you a better understanding of your rights to information access and privacy, the provincial government's obligations under the Act, and the process of accessing information. Again, while the two Acts are not the exact same, this Alberta course is a useful resource.
Canadian Legal Information Institute Database [CanLII] - CanLII is a non-profit organization managed by the Federation of Law Societies of Canada. CanLII's goal is to make Canadian law accessible for free on the Internet. This search engine is not limited to access and privacy matters but provides access to many court decisions or past decisions of the federal and provincial Information and Privacy Commissioners of Canada.

Privacy and Access Council of Canada - Conseil du Canada de l'Accès et la vie Privée (PACC-CCAP) - PACC-CCAP is a national association in Canada dedicated to the access and privacy community in both the private and public sectors. Its services are relevant to members of the profession, as well as to others who are interested in understanding Canada's access and privacy landscape. PACC-CCAP provides independent, unbiased and substantive professional certification of access and privacy professionals and offers a range of education, training and professional development opportunities.
